When Success Leads to Failure: Lessons from NASA [Presentation]

20 years ago today, the Space Shuttle Columbia disintegrated as it re-entered the earth’s atmosphere, killing all seven crew members. A few months prior, NASA had been named the “Best Place to Work” amongst all government agencies. Phillip was asked to lead the culture change initiatives following the accident, trying to uncover the missing link. 20 years later to the day, we will take an in-depth look at the tragedy and how it shaped ideas about culture. This keynote session discussed the key events, approaches and lessons learned during that challenging time.
