Yet often the real drag on performance can found in between the main steps, - the "micro" or hidden processes - where manual work and inefficient or poorly integrated applications slow people down. I am a big fan of Cross-Functional Diagrams or Swim lane process maps. My experience is that the process pain can be often found at the hand-off's between one Swim lane function and another.

Yes, I agree that the "in between gaps" have significant negative contribution. They need to be addressed with equal priority with the objective of eliminating the gaps through process and boundary changes.
